AFAIK, you don't have to run fs-setup when upgrading.  That is only for
the initial installation.  As for your error, you still have the old
indicies from your previous database and either need to drop the indicies
or the whole database.  That is, if you dropped all your tables in an
attempt to start from scratch.  If not, you shouldn't be running fs-setup.

I thought MySQL didn't work with 1.4.0pre.
