Which version of Freeside are you using and which Batch Customer Import Format did you select?

Here is an example csv record that works on version 4.1 with format "Extended plus company"

"1234","Word of Mouth","LastName","FirstName",,"123 Test St.",,"Test City","CA","92373","us","","8005551234",,,,,,,,,"",,,,"billtest+1234 at example.net",,,
