[freeside-users] I cannot see 'cash' in the '* Billing type' drop down.

I don't think MCRD will do exactly what you want.  It doesn't run the 
card through Freeside, it just records the payment.  "Manual" indicates 
the card is run "manually" (i.e. through a POS terminal).
