[freeside] First Installation

The only issue I had with 4.1 ....  is that in the docs  (1.5.0pre3) when
explaining how to add access to mysql   ...  you need to set
Create_tmp_table_priv   to Y  for the freeside user ... it doesnt mention
that right now. Other than that .... everything else was straight forward
... sorry I forgot to mention that in the last message.
