BankOfAmerica Processing Error

We have Online::Payment::BankOfAmerica configured, and almost working.  We
can signup customers and the CC information is sent to BofA and processed
correctly.  The problem is that the billing on Freeside is failing with the
following error under Failed Billing Events:

failed: IOC_Duplicate_Flag=No at
/usr/lib/perl5/site_perl/5.6.1//Business/OnlinePayment/BankOfAmerica.pm line
181.

I have called BofA, and ioc_duplicate_flag=no tells us that that settlement
request has a unique handshake ID, which is a good thing.
