If I'm not mistaken, for B::OP::BankOfAmerica, you need:

BankOfAmerica
<login>
<password>
Authorization Only
merchant_id
<merchant id>

You might also want "Authorization Only, Post Authorization" instead of 
just "Authorization Only".
