Problems with Freeside and DBM

Hi List,
 
We have installed all the required software listed below for Freeside
but whenever we try to run the new customer script from the browser I
get the following error. 
 
Can't get REMOTE_USER! for cgi CGI=HASH(0x804b5a4
 
All software seems to install ok with no errors. The archives point us
to Authentication problems with DBM. 
 
Does anyone have any idea whats going here?
