[freeside-users] selfservice error unknown package

When a customer is in the selfservice.cgi under "Setup my service" and
attempts to change packages using the "change" link, it shows a list of
packages. When the customer selects a replacement package the system returns
an error of "Unknown Package". How do I fix this?
