I've had this issue a few times as well.  Since I was testing a new
setup, I installed from bare metal and got it working.  I'm using
debian, and added to the wiki

I am going to attempt to isolate what's different between the two
setups I have. (working, and not).  I'll let anyone know what my finds
are, if anything significant.  One thing I cheated on, was to
temporarly allows postgresql to 'trust' anything in the pg_hba.conf. 
This is one thing I might not be able to isolate, because I'm not sure
if I tried that in the old setup. (I could however, boot into that
exclusivley and try there...)
