I do believe this would occur if you accessed a web page that is
not password protected, or you did not limit GET _and_ POST, i.e.
<Directory "/var/www/freeside">
AllowOverride All
Options Includes FollowSymLinks ExecCGI
AuthType Basic
Authname Freeside
AuthUserFile /usr/blah/blah
<LIMIT GET POST>
require valid-user
</LIMIT>
</Directory>
