Trouble

This looks like a problem with your MySQL installation.  The cust_main.frm
file is from MySQL.  It doesn't come with the distribution, and infact is
specific to MySQL.  On my system, it lives in /var/lib/mysql/freeside. 
Have you installed _and tested_ MySQL, DBI and DBD::mysql?  Have you
created the freeside database and set appropriate permissions?  Have you
run fs-setup to make the tables?
